That said, how long since a fuel filter change and / or a fuel pressure test?

Check the essentials bro use gauges and codes as a guide .. Fuel pressure test is a must, as already mentioned , needs fuel spark compression and vacuum find which one is giving u a problem with basic tester kits and the u can open it can of worms from there

-Vacuum gauge to intake somewhere
-Compression test ALL cylinders
-Fuel pressure tester to the test port on the fuel rail
-Battery self explanatory just make sure ur reading matches the gauge
-oil pressure u will have to check on a load .. The tester replaces the oil pressure sender so just make sure to take mental bits of the gauge fluctuation before the test

Also the coolant temp sensor I. The side of your block controls part of the fuel system ..

You mentioned u replaced a radiator ..

Try unplugging that while its acting up and if it runs differently pull the sensor and look at it , if it just looks like copper wire it's melted should have a black or white Teflon hardened gel stuff on it ..
